Informed Consent for Waxing Services
I understand that waxing services involve the removal of hair using wax applied to the skin. I acknowledge that the following temporary reactions are normal and may occur: redness, sensitivity, minor irritation, bumps, and tenderness in the treated area lasting up to 48 hours.
I understand that more serious reactions, though uncommon, may include lifting or removal of skin, bruising, burns, ingrown hairs, infection, or hyperpigmentation, particularly if contraindications are present or aftercare instructions are not followed. I agree to disclose all relevant health information accurately and to follow all aftercare instructions provided.
I confirm that I am 18 years of age or older and that I have disclosed all health conditions, medications, and contraindications to the best of my knowledge. I understand that Double Cleanse Skin reserves the right to decline service if any active contraindications are identified.
I release Double Cleanse Skin LLC and Ashley Moran from liability for reactions that occur as a result of undisclosed health information, failure to follow aftercare instructions, or conditions outside of their control.
Histamine Reaction Advisory
Waxing can trigger a histamine response in some clients. This is a natural physiological reaction, not an allergic reaction, and occurs when the skin releases histamine in response to hair removal. Symptoms may include redness, raised bumps, hives, itching, or a feeling of heat in the treated area. These reactions are typically temporary and resolve within a few hours.
Clients who are prone to histamine reactions, have sensitive skin, or have a history of allergies may experience a stronger response. Taking an antihistamine such as Benadryl one hour before your appointment can help reduce the severity. Please inform Ashley before your service if you are prone to histamine reactions.
I understand that a histamine response is a known and common side effect of waxing. I acknowledge that Double Cleanse Skin LLC is not liable for a histamine reaction that occurs as a result of my service, so long as proper techniques and professional products are used.

Photo and Video Release (Optional)
I give permission for Double Cleanse Skin LLC to photograph or record before and after images of my waxing results. These images may be used for portfolio, social media, educational, or promotional purposes. My personal identity will not be disclosed without my additional written consent. This release is entirely optional and will not affect your service in any way.
